eCard

We're happy to offer affordable prices to our uninsured patients, or patients with insurance which only covers catastrophic conditions.  You must attest that you do not have any insurance. The eCard is not health insurance.

Your discounted cost: $89 for office visits. Usual office visits are $140-183.

If patients obtain insurance during the year, eCard membership will be discontinued effective the date of insurance activation.

X-rays, procedures such as suturing, splinting, incisions; immunizations, durable equipment, medications are additional. Testing, lab and other procedures performed by outside facilities will be billed separately by the providing institution. Occupational medicine services and school physicals are charged at standard rates.

Gift Card

New Patients: present our ad or postcard after your first office visit, and receive a $20 gift card to Walgreens or Rite Aid.*

*offer expires 12/1/2010. Can only be used for patients with office visits 4/10/10 - 7/1/2010, who have not been seen in 3 years prior to 4/1/10. Card is only for patients who see a physician, PA or NP, and are paid in full or are seen under current participating health plan. Lab work visits, prior employee health arrangements, drug screens, prior contracted physicals all excluded. Card will be issued to guarantor only.

Friends and Family

New patients, referred by current patients, may receive a $20 gift card to Walgreens, Rite Aid or Target.  Not to be combined with "New Patients" offer above.

Referring patient will also receive a $20 gift card, provided he/she is not a minor, and has been seen as a non-contracted patient in our office in the 1 year prior to 4/1/10. 

All decisions regarding gift card eligibility are by eMedical Offices and are final.
